Bring this checklist to prescriber or pharmacy visits so the most important medication questions don’t get lost. You don’t have to ask everything — just mark what matters most for this visit.

Understanding each medication

Can you remind us what this medication is for and how you expect it to help at this point?

How will we know if it’s working, and over what time frame?

Are there side effects we should watch for that would mean we should call you sooner?

Checking for lower-cost or simpler options

If cost is a concern, are there lower-cost alternatives that would still be appropriate?

Are any of these medications on a higher tier or not preferred by our plan that you know of?

If we can’t change this medication, are there others on the list that could be simplified or combined?

Looking at the whole list

Looking at the whole list, are there any medications you’d consider stopping, reducing, or changing, if things are stable?

Are there any interactions with daily life we should be thinking about (falls, confusion, sleep, appetite)?

If things change suddenly, which medications (if any) should we stop or adjust right away, and who should we call first?
