None of the entries above are recommendations. They’re examples of how you might summarize what you find in state and federal databases.

How to read what you’re seeing

Numbers and flags don’t tell the whole story, but they do tell you where to ask harder questions.

A facility with some past issues but clear improvement can be very different from a facility with ongoing, unresolved problems.
Ownership and administrator stability often correlate with culture and follow-through.
Complaint volume matters, but so does how quickly issues are addressed and closed out.
Staffing numbers are a clue, not a guarantee — you still need to visit and observe.

Always pair the data with in-person visits, conversations, and what you see and feel on the ground.

Red flags vs. “yellow” flags

Not every imperfection is a deal-breaker. This is one way to think about different signals:

Red flags: repeated serious deficiencies, ongoing enforcement actions, major staffing gaps, or patterns of neglect in complaints.
Yellow flags: minor repeated issues, moderate complaints about communication or delays, staffing that’s borderline but improving.
Context matters: a facility that openly explains past issues and shows what changed may be safer than one that deflects or minimizes.

Use the red/yellow framing to decide which facilities need deeper questioning — not to automatically rule them in or out.

What to do after using this dashboard

Once you have a short list from Facility Insights:

Schedule tours and unannounced visits at different times of day.
Bring a written list of questions based on the patterns you saw.
Ask staff directly about turnover, enforcement, and how they responded.
Talk with your primary care physician, specialists, or care manager about whether choices match the level of care needed.

The point of this dashboard is not to be “right” on its own — it’s to help you collect, organize, and then share what you’re seeing.

In later phases, this page can support saving your facility list and exporting it as a simple PDF or spreadsheet. For now, you can copy this structure into your own notes or spreadsheet and bring it to appointments with doctors, care managers, and family members.
