Facility Insights pulls together patterns from inspections, complaints, staffing, enforcement actions, and ownership changes so you can decide where to look closer — and what to ask when you get there.

Rough in your search area

This dashboard is designed as a planning surface. In the live version, filters will pull from public data sources; for now, you can use it as a structured worksheet as you research facilities online.

In a future build, these filters can pull directly from CMS and state-level datasets. For now, use this as a template to capture what you find across multiple sites.

The point of this dashboard is not to be “right” on its own — it’s to help you collect, organize, and then share what you’re seeing.

In later phases, this page can support saving your facility list and exporting it as a simple PDF or spreadsheet. For now, you can copy this structure into your own notes or spreadsheet and bring it to appointments with doctors, care managers, and family members.
